The CPC+ model, set to launch Jan. 1, 2017, consists of two payment tracks in which primary care practices either get a monthly fee in addition to Medicare fee-for-service payments, or a monthly fee and a hybrid of Medicare fee-for-service payments and up-front primary care payments. Related CPC+ is one of the few opportunities for primary care providers to participate in a MACRA-recognized advanced alternative payment model.
