New primary care model set to launch in 2017

Practices have through Sept. 15 to apply if they wish to participate in the Comprehensive Primary Care Plus model.

Applications will be accepted through Thursday, Sept. 15, for the Comprehensive Primary Care Plus (CPC+) model.

The CPC+ model, set to launch Jan. 1, 2017, consists of two payment tracks in which primary care practices either get a monthly fee in addition to Medicare fee-for-service payments, or a monthly fee and a hybrid of Medicare fee-for-service payments and up-front primary care payments.

CPC+ is one of the few opportunities for primary care providers to participate in a MACRA-recognized advanced alternative payment model. The new model is expected to include up to 5,000 practices.
